INFO NEEDED TO PREPARE A U.S. INCOME TAX RETURN

 

In order to prepare your tax returns properly and efficiently, we will need the following:

1.  Names, as they appear on Social Security Card for all members of the family. Copies of Social Security cars are preferred.

2.  Citizen Status, self, spouse, children (US citizen, Non-Resident Alien, Israeli citizen, other).

3.  Dates of Birth for all members of the family.

4. Social Security Numbers for all members of the family. Please supply copies of Social Security cards.

5.  Address, including Zip Code.

6.  Phone Number(s) (home, cell and work).

7.  E-mail address(s) (home and work).

8.  Date of Aliyah.

9.  Occupation(s).

10.  Income  - if employed -Tofes 106.  If self-employed, your profit/loss schedule from your Israeli tax return. If you received maternity pay or unemployment from Bituach Leumi, we require the year end ishurim.

11.  Interest and Dividends - If you have received interest, dividend or sales of securities we require the year end Form(s) 1099 issued by the payor. If you received interest, dividends or sold securities in Israel, we require Israeli Form 867 from your bank or the Israeli tax return.

12.  Rental Income - the income and expense amounts and original cost of the property, date of purchase plus improvements.

13. Pension Income - all 1099's from the U.S. and relevant forms from Israel.

14. If you have a United States bank account, or a family member is willing to let you use theirs, we require the Bank Account Number and Bank Routing Number. It is a good idea to have any refund directly deposited into a U.S. bank account, as there has been a rash of check theft both in Israel and the U.S.

15.  All Other Income,  deductions, credits and other information not included above.
